Special Events Request Form

Welcome!  If you would like to schedule an event , please fill out the form below and then click "Submit".  After you submit the form, you will be redirected to a page to confirm your event information.

After confirmation and final submittal, your information will be forwarded to our Special Events Coordinator for review, and you will receive a confirmation e-mail that your request was received.

PLEASE NOTE!  We do not have a traditional "fire hall" available for rent.  Any events with attendance in excess of approximately 25 people may need to be held off-site.

Due to numerous event commitments and ongoing training requirements, F.D.M.T. prefers a minimum of three weeks notice to schedule events.  Completion of this form DOES NOT guarantee that the department will be able to accomodate your request.  An F.D.M.T. representative will contact  you to discuss the specifics of your event upon receipt of your request.
